This is a 12-month membership which will automatically renew monthly unless cancelled in advance.
Cancellation must be presented by phone or email with a 30-day notice.
Membership Fees are Non-Refundable.
This membership fee recurs monthly unless cancelled prior to end of term.
12-month minimum commitment is required, otherwise a cancellation fee of $149 will be charged.
All included services are rendered in-office and an appointment is required.
Upgrades may be added at the time of service for an additional fee.
We reserve the right to refuse membership to anyone at our discretion.
Member discounts are not combinable with other promotional discounts.
The membership is non-transferrable.
Complimentary monthly services must be completed within 30 days of the membership run date.
