Terms and Conditions:

Membership and Billing

• This is a 12-month membership with a minimum commitment of 12 months. Membership fees are billed monthly at the rate of the selected tier.
• After the initial 12-month term, the membership will continue on a month-to-month basis and will automatically renew each month unless cancelled.
• All membership fees are non-refundable.

Cancellation Policy

• Cancellation must be submitted by phone or email with at least 30 days’ notice prior to your next billing date.
• If membership is cancelled before the 12-month minimum commitment is completed, a cancellation fee of $149 will be charged at the time of cancellation.

Services

• All included services are performed in-office and require a scheduled appointment.
• Upgrades and add-ons may be purchased at the time of service for an additional fee.
• Complimentary monthly services must be used within 30 days of your monthly billing date. Unused services do not roll over.

General Terms

• Member discounts cannot be combined with other promotional offers or discounts.
• Membership is non-transferable and is valid for the named member only.
• Gallaher Plastic Surgery and MedSpa reserves the right to refuse or terminate membership at our discretion.
